Responsibilities

The Transplant Coordinator is a member of the team responsible for coordination of the clinical process from patient referral, evaluation, acceptance, treatment and follow-up. Incumbent serves as a resource to staff and multi-disciplinary team and community caring for program patients. Demonstrates knowledge and skills in the care of patients from adolescent to adult, and will play a role in the holistic evaluation of patient(s). Skills in the area of critical thinking, interpersonal communication, conflict management, problem solving, autonomy and decision-making are highly desirable. Maintains program compliance and QAPI. Complies with regulatory, governing and other external agencies requirements in the area of quality, data reporting, ac with the program Medical Director and administrator, provides

for timely data input and management. Assists in developing policies and procedures, order sets and pathways to improve overall quality of care and program development. After hours and weekends required to meet patient and program needs. On call duties as assigned.

• Organizes the clinical program

• Develops, implements, reviews, revises and maintains policies, procedures, and standards of care for patient population

• Develops, implements, and maintains ongoing continuous quality improvement activities for the clinical program.

• Serves as resource to hospital staff regarding patient care.

• Coordinates the patient evaluation process.

• Performs initial assessment of patient and family according to protocol for acceptance into program

• Coordinates team members in patient selection process.

• Provides education to patient and family regarding clinical processes.

• Maintains complete documentation on patient and family.

Qualifications

MINIMUM

Graduate of accredited school of nursing.

Three years experience with patients in a related setting and/or three years experience in an intensive care, acute care, telemetry or medical/ surgical setting.

Registered Nursing License (RN) in state of practice

Working knowledge of regulatory compliance and survey processes.

Joint Commission and CMS standards.

Working knowledge of QAPI process.

Strong verbal, written communication skills.

Proficient computer skills.

Performance improvement tools and methodologies such as PDCA, Six Sigma, Lean, etc.

Pay Range

$42.69 - $61.91 /hour
